Field Trip Form

*note Thursdays and Fridays are preferred*

I agree to inform each parent that they will be charged $10.00 (tax included) per person ages 3+ the morning of the field trip
A tax exemption certificate is due no later than the morning of your vistit
Accurate number per current student enrollment- do not guess!) No refunds given. Fritzler Farm Park will give you the wristband(s) for student no-shows. They can be used any remaining weekend during the 2018 maze season. Students who have moved will still be given a wristband. Teacher can use it as an INCENTIVE PROGRAM at your discretion
I will share the field trip guidelines with all students, teachers and chaperones (SEE BUTTON BELOW)
